What a pharmaceutical QMS is

A pharmaceutical quality management system (QMS), called a pharmaceutical quality system (PQS) in ICH Q10, is the management system that directs and controls a drug company with regard to quality. It goes beyond GMP compliance: GMP sets minimum requirements for each batch, while the PQS makes sure the whole organization learns, improves and keeps processes in control across the product lifecycle.

ICH Q10, adopted by FDA, sets three objectives: “Achieve Product Realization,” “Establish and Maintain a State of Control” and “Facilitate Continual Improvement” (FDA, ICH Q10).

Leadership is a requirement, not a slogan

ICH Q10 is direct about who owns quality: “Senior management has the ultimate responsibility to ensure an effective pharmaceutical quality system is in place,” and “Senior management should be responsible for pharmaceutical quality system governance through management review” (FDA, ICH Q10). It also covers partners: the company’s responsibility “extends to the control and review of any outsourced activities.”

FDA inspection data shows why this matters. The top FY2025 drug citation was 211.22(d), quality unit procedures, and the analysis behind that count describes a quality unit “not consistently functioning as the governing authority mandated by Part 211” (GMP Insiders). Weak leadership support usually sits behind that finding.

Measuring maturity: FDA’s QMM program

FDA’s Quality Management Maturity (QMM) program looks past minimum compliance. In February 2026, FDA announced a third year of its voluntary prototype assessment program, selecting nine establishments; the program “evaluates how effectively drug manufacturers monitor and manage quality and quality systems” and “does not impose new regulatory requirements” (Arnold & Porter). Even if you do not participate, QMM’s questions are a useful self-assessment.

Building the PQS

  1. Quality manual and policy that state scope, structure and management commitment.
  2. Process performance and product quality monitoring: defined indicators, trending and annual product review.
  3. CAPA that reaches root cause and checks effectiveness.
  4. Change management with risk-based assessment, including for suppliers and contract sites.
  5. Management review at a fixed cadence with decisions and resources recorded.
  6. Knowledge management that carries learning from development to commercial production.
  7. Quality risk management applied consistently, per ICH Q9(R1).
  8. Outsourced activity oversight through quality agreements and audits.

Pharmaceutical QMS checklist

  • Quality manual reflects ICH Q10 elements and enablers
  • Senior management review held on schedule, with decisions and resources
  • Product quality indicators trended; annual product reviews complete
  • CAPA effectiveness verified
  • Change management covers suppliers and contract sites
  • Knowledge management process defined
  • Quality risk management aligned with ICH Q9(R1)
  • Outsourced activities controlled through agreements and audits
  • QMS software validated and integrated with quality metrics
  • Self-assessment against QMM concepts completed

Assess the PQS against ICH Q10 with evidence

A PQS is only as strong as the records that show it working. AskDona reviews quality manuals, management review records, CAPA and change control files against ICH Q10, 21 CFR 211 or your own standards, flags gaps and weak links between elements, and cites the passage behind every finding.

Run a sample PQS gap assessment on your quality manual and one quarter of management review records. AskDona shows the evidence; your leadership decides.

FAQ

What is a pharmaceutical QMS? The management system that directs and controls a drug company with regard to quality, often built on ICH Q10.

What are the four elements of ICH Q10? Process performance and product quality monitoring, CAPA, change management, and management review (FDA).

Is ICH Q10 mandatory? It is guidance, not regulation, but FDA adopted it and inspectors expect its concepts; GMP rules themselves are mandatory.

What is FDA’s QMM program? A voluntary program that assesses how mature a drug manufacturer’s quality management is (Arnold & Porter).

What should pharma QMS software include? Deviation, CAPA, change control, document control, training, supplier management, metrics and Part 11 controls.
