CAPA management is a system, not a form
CAPA management is the program that makes corrective and preventive action work across an organization: how issues enter, how they are prioritized, who investigates, how actions are tracked and how effectiveness is proven. Most CAPA failures are management failures. FDA warning letters repeatedly cite investigations that do not reach root cause and CAPAs that arrive too late; one 2025 letter stated simply, “Your response is inadequate because your CAPA was not timely” (FDA warning letter, Apotex).
CAPA remains a top inspection risk. An analysis of FDA’s FY2025 observation data found inadequate CAPA procedures were the most cited device observation, with 279 citations (GMP Insiders), and corrective actions ranked fourth among early QMSR inspection observations in 2026 (RAPS).
For CAPA software requirements specific to medical devices under the QMSR, see CAPA Management Software for Medical Devices: What Changed Under QMSR.
The building blocks of CAPA management
| Element | What good looks like |
|---|---|
| Intake | All quality data sources feed one process: complaints, deviations, audits, nonconformances, trends, inspections |
| Triage | Risk-based criteria decide whether a CAPA is needed and how deep the investigation goes, consistent with ICH Q10’s principle that effort “should be commensurate with the level of risk” (ICH Q10) |
| Investigation | Structured root cause analysis with evidence |
| Action planning | Actions address root causes, with owners, due dates and verification |
| Effectiveness | Measurable criteria checked over time |
| Oversight | Trending, KPIs and management review |
CAPA KPIs worth tracking
| KPI | Why it matters |
|---|---|
| Average age and overdue CAPAs | Timeliness is a recurring warning letter theme |
| Time to complete investigation | Long investigations delay protection |
| Recurrence rate | Repeat issues signal weak root cause analysis or ineffective actions |
| Share of CAPAs with retraining as the only action | FDA’s quality surveillance tracks “The proportion of FAR having CAPA SOP changed/Retraining as the only outcome” (FDA OQS) |
| Share of root causes coded as human error | FDA tracks “The proportion of root causes associated with human error” (same source) |
| Effectiveness check pass rate | Shows whether CAPAs actually work |
There is no regulatory closure deadline. Vendor AssurX notes some organizations target closure within 60 days but that neither ISO 9001 nor 21 CFR defines such timelines (AssurX). Set targets by risk and justify them.
What CAPA software should do
| Capability | What to look for |
|---|---|
| Unified intake | Links from complaints, deviations, audits and nonconformances into CAPA without re-keying |
| Risk-based workflow | Configurable triage, escalation rules and approval routing |
| Investigation support | Templates for 5 Whys, fishbone and FMEA; attachments; links to evidence |
| Action tracking | Owners, due dates, reminders, escalation of overdue items |
| Effectiveness checks | Scheduled checks with predefined criteria and results |
| Change and training links | Actions that change procedures trigger change control and retraining |
| Trending and reporting | Dashboards for KPIs, recurrence and management review |
| Compliance controls | Part 11 audit trails, electronic signatures and access control |
| Validation support | Vendor evidence for a risk-based validation approach |
For device makers, FDA’s Computer Software Assurance guidance, finalized in September 2025 and updated in February 2026, describes “a risk-based approach to establish confidence in the automation used for production or quality management systems” (FDA). Drug manufacturers validate under their own CGMP requirements.
CAPA automation and AI: where they help
Automation removes hand-offs: routing, reminders, escalation and record linking. It is predictable and easy to validate.
AI helps with the reading-heavy parts: finding similar past events, summarizing investigations, checking whether a root cause is supported by evidence, and spotting trends across text records. FDA anticipated this in 2023, noting that “AI can be used to examine consumer complaints and deviation reports” and “can be used to proactively monitor manufacturing operations for trends” (FDA CDER discussion paper).
The guardrails are now explicit:
- FDA expects human review. An April 2026 warning letter found a firm had “used AI to create drug product specifications, procedures, and master production or control records,” and stated AI output “must be reviewed and cleared by an authorized human representative of your firm’s QU” (FDA warning letter).
- EU GMP is drawing lines. The draft Annex 22 says generative AI and LLMs “should not be used in critical GMP applications” and keeps qualified personnel responsible for outputs in a human-in-the-loop model (European Commission draft).
In practice: let AI suggest and cite, let people decide and sign.
CAPA software selection checklist
- Current CAPA process mapped, with pain points and KPIs baselined
- All quality data sources can feed CAPA
- Risk-based triage and escalation configurable
- Investigation tools and evidence links supported
- Effectiveness checks scheduled with predefined criteria
- Change control and training integration available
- Part 11 controls verified where required
- Risk-based validation approach agreed with vendor evidence
- AI features cite sources and require human approval
- KPI dashboards support management review
Strengthen CAPA quality, not just CAPA speed
Software can move a CAPA faster; it cannot tell you whether the investigation is good. AskDona reviews CAPA records and investigation reports against your procedure, ICH Q10, ISO 13485 or 21 CFR expectations, flags unsupported root causes, missing scope assessment or weak effectiveness checks, and cites the passage behind every finding.
Run a sample CAPA record assessment on one recent CAPA. AskDona shows the evidence; your quality team decides.
FAQ
What is CAPA software? Software that manages corrective and preventive actions from intake to investigation, action, effectiveness check and closure.
What is CAPA management? The overall program for running CAPA consistently, including triage, investigation, tracking, effectiveness and oversight.
Can AI write CAPA investigations? AI can draft and suggest, but FDA expects qualified people to review and approve AI output in GMP work (FDA).
Does CAPA software need validation? In regulated industries, yes, proportionate to risk.
How long should a CAPA take? Regulations do not set a fixed time; define risk-based targets and justify delays.
